Service: Psychology and Life Coaching Sessions

1. Consent for Services

I, the undersigned client, understand and consent to receive psychology and/or life coaching services from Yousef Yousef / Driven Life Coaching LLC. I acknowledge that:

• The services provided are intended to support my personal development, mental well-being, and life goals.
• I understand that coaching is not a substitute for medical, psychiatric, or therapeutic treatment and does not replace professional mental health care if required.
• I agree to be open and honest during sessions to achieve the best outcomes.

2. Assumption of Responsibility & Liability Waiver

• I understand that all outcomes of the coaching and psychological sessions are my personal responsibility.
• I release Yousef Yousef / Driven Life Coaching LLC from any liability or claims related to the outcomes of the services provided.
• I agree that no guarantees or promises have been made regarding the results of these services.

3. Payment and Cancellation Policy

• All payments for sessions are non-refundable.
• If I cancel a scheduled session, I acknowledge that I will not receive a refund.
• Rescheduling may be allowed at Driven Life Coaching LLC's discretion if notice is given at least 24 hours before the session.

4. Confidentiality

• I understand that all information shared during sessions will remain strictly confidential, except where disclosure is required by law (e.g., imminent harm to self or others).

5. Acknowledgment

By signing this agreement, I confirm that I have read and understood this policy and consent form. I agree to the terms outlined above.
